Residential		Use this form to initiate new service for an Existing Meter on a Residential Property.
Commercial or Business		Use this form to initiate new service for an Existing Commercial or Business use Meter.
New Meter Install / Line Extension		Use this form to initiate a quote regarding New Meter Installs or Line Extensions, please be sure to have the parcel number and/or service address available.
Plant Standpipe		Use this form to initiate new service for our Plant Standpipe.

Service Establishment Fee	$40.00
New Meter (if required)	(additional)
Deposit *	$90.00		* Deposit refundable after the 1st year of service, unless any payment is late.  Redeemable only as a credit towards water service on your account.
Total due in full to turn on water:	$130.00
